Offering Support

The Center for Hope and Healing offers support to people in our community who are struggling with mental, medical and social health problems. The Center was created to give patients access to providers who can help stabilize their health and their lives.

Who We Serve

We treat anyone with a mental health issue and at least one other medical or social health problem. Our patients may suffer from bipolar disorder, schizophrenia, severe major depression or another severe mental illness that has not been fully diagnosed. In addition, the patient has:

  • A significant social health need (such as housing, food insecurity, etc.) OR
  • An uncontrolled acute or chronic illness (such as diabetes, heart disease, asthma, etc.).

Our services include:

  • An appointment with a psychiatric provider.
  • A physical exam with a focus on preventive care and control of chronic medical conditions (with co-management of conditions with the primary care physician).
  • A complete evaluation by a social worker, who helps patients access community, state and federal resources.
  • A home visit to gain an understanding of the home and neighborhood environment.

Our specialists work together to quickly stabilize patients’ health and then put long-term support in place with other providers and programs. We provide flexible, responsive care in a respectful, caring and supportive setting.

Patients can be referred by doctors or community-based programs. Priority is given to patients being seen within the ChristianaCare.

Convenient Care with a Supportive Team

We see patients at one convenient location in Wilmington Hospital, as well as appointments via telephone. Our team members include:

  • Behavioral health therapist.
  • Primary care provider.
  • Psychiatric provider
  • Nurse/case manager.
  • Social worker.
  • Community health worker.

Patients can be referred by doctors or community-based programs. Priority is given to patients being seen within the ChristianaCare.
