When you are facing the challenge of cancer, finding the right care and services you need can seem overwhelming. The Helen F. Graham Cancer Center & Research Institute Cancer Care Management team works with you and your doctor to help support your care needs, both in and out of the hospital.
Through Cancer Care Management, you can be connected to our team of Nurse Navigators and Social Workers. Our objective is to provide high-quality care coordination, patient-centered care that focuses completely on you.
Our nurse navigators are clinically trained specialists with expertise in supporting patients diagnosed with cancer. Nurse navigators can support coordination of care between you and your care team, provide education about the cancer diagnosis and review support services available to ease anxiety as you navigate the healthcare system.
Living with cancer may mean that you will have to deal with added financial, physical, and emotional concerns.
Our oncology social workers are trained in how to help people manage these life changes and can give you guidance and support as you face these challenges.
Both our nurse navigators and social workers work with the entire team of cancer professionals, including physicians, nurses, social workers, dietitians, genetic counselors, chaplains, cancer researchers, the oncology data center, librarians, health psychologist, complementary-medicine providers and community-resource providers.
Through our team we can connect you to services at the Helen F. Graham Cancer Center & Research Institute as well as resources within the community.
