Care Management Program
In partnership with xG Health Solutions, an affiliate of Geisinger Health System, Christiana Care offers employees and their dependents the assistance of an experienced Care/Health Manager.  These Health Managers focus on CCHS members with one of several chronic conditions.  They develop a relationship with the member, the member’s family and support structure and work in collaboration with CCQP providers to help the member navigate through the health care system in order to achieve the most optimal health outcomes.
Over these first several months, many CCHS members have experienced positive results in their personal interactions with the Care Manager team.  Members report having a better understanding of the care process and also being more involved and in greater control of their care.

If you notice anything about your group or physician(s) information that needs to be updated or is missing, please contact your provider relations representative for assistance

Submitting Flu Vaccine Claims
Please note that if you are accustomed to billing codes beginning with a “906” for the flu vaccine, you should be using codes beginning with a “Q” to avoid denials. Please click here for more information.

Imaging for Members
While precertification is not required for imaging services, certain high-end imaging procedures such as CTs and MRIs will go through a concurrent review process. The ordering provider may be contacted by a Radiologist to further discuss the case. As a reminder, all Christiana Care Imaging facilities are participating providers for members’ imaging needs.

Notification of Providers Joining or Leaving Your Group
If your practice has providers that are joining or leaving the group, please remember to let your provider relations representative know as soon as possible and provide the effective date when you do so. If you are unsure as to who is your representative, please call us at 302-623-7959.

Navigating NaviNet

Please be advised that if you see NIA (National Imaging Associates) as part of Geisinger’s screen in NaviNet, please disregard this for CCQP members.

Member Identification Cards
Please be advised that not all Christiana Care employees and their dependents are members of Quality Partners. If an employee resides outside of Delaware, they and their dependents are still covered under Highmark and as such will still carry a Highmark member ID card. All employees who live in Delaware and their dependents are covered by Quality Partners. Please be sure to confirm the patient’s address when they present their member ID card.
