For patients without a primary care provider (PCP), you can now refer them to the PCP in the Oncology Express Unit and be assured of management and follow up by a PCP with a focus on oncology patients.
Here’s how it works:
Referring clinician or member of the practice calls the Oncology Express Unit to make the referral.
For patients who do not currently have a primary care provider only.
Patients will be followed by the Unit’s PCP until completion of treatment by a radiation, surgical or medical and/or hematologic oncologist.
After treatment patient will be connected to a community primary care provider.
Primary care providers play an important role in their patient’s cancer care. PCPs can address issues such a hypertension and hyperglycemia that might prevent or delay treatment. Patients without a PCP oftentimes seek management of issues of the primary health domain from their oncologist.
