Hospital Violence Intervention Program

The HAVI-certified Hospital Violence Intervention Program (HVIP) at ChristianaCare is led by Community Health Workers (CHWs) in our Community Health Impact department in collaboration with the Trauma Department and Emergency Department. CHWs provide support to patients impacted by community violence post-discharge to help improve health outcomes.

Support with post-discharge follow-up care and access to ongoing health services.

For Patients

The HVIP CHWs can:

  • Assist with scheduling and accompanying you to medical appointments.
  • Connect you with community resources and services.
  • Partner with you to identify and achieve your goals.
  • Provide ongoing support throughout your recovery and wellness journey.
Eligibility:
  • 13+ years old 
  • Delaware resident 
  • Impacted by community violence and treated at a ChristianaCare location
  • Injury had to take place within 6 months
