In-Home Care

Our skilled and experienced nurses, home health aides, social workers and therapists will come to your home to provide medical nursing, rehabilitation and social services, as well as personal care to help your loved one with activities of daily living. We’ll help maintain routines and simplify tasks at home, to make your loved ones comfortable.

Skilled Nursing Care

Infusion Therapy

For some conditions, medicine, nutrients, and other treatments must be given via intravenous (IV) injection. This type of treatment is also called infusion therapy. Our specially trained infusion therapy nurses provide comfort and healing, including:

    • IV site care.
    • Administration and instruction on IV therapy.
    • Drawing and reporting lab values.
    Wound Care

    Wounds that are not treated properly can take longer to heal and cause other health problems. ChristianaCare HomeHealth certified wound care specialists will come to your home to be sure you or your loved one receives safe and effective treatment that manages pain and promotes healing.  

    Our wound care experts are experienced in managing a wide variety of skin care needs, from minor skin irritations and injuries to complicated, complex wounds. Plus, we document progress with digital imagery, carefully monitor changes in your healing process and adjust treatment as necessary. Count on us to come to you and provide:

      • Instructions on how to care for your or your loved one’s wounds.
      • Wound treatments that manage pain, prevent infection and promote healing.
      • Follow-up with your physician to assess progress.

      After Hospital Care

      When you or your loved one needs help after coming home from the hospital, ChristianaCare HomeHealth will come to you to provide the specialized support you need to help in your recovery. Our nurses, physical therapists and other in-home health professionals, social workers and home health aides are specially qualified to help you or your loved one recover, safely at home. 

      • A full assessment of your physical ability and care needs.
      • Individualized care plan and care coordination with your doctor.
      • Pain management support.
      • Physical, occupational and speech therapy.
      • IV therapy.
      • Help with and instruction in wound care.
      • Help in managing medications.
      • Guidance, education and assistance in following discharge instructions.
      • Instruction to safely use a walker or cane to help with mobility.
      • Coordination for help with meals, transportation, applying for medical assistance, and support to stay comfortably at home.

      HomeHealth Aides

      If you or a loved one is recovering from an illness, injury or hospital stay, or dealing with a chronic illness, disability or intellectual impairment, turn to ChristianaCare HomeHealth for help. Our home health aides can help you or your loved one maintain independence by providing personal care assistance – such as bathing, dressing and grooming – and helping with meals, appointment management, and medication schedules. Our aides offer companionship with therapists and other medical staff to provide complete patient care.

      Medical Social Workers

      Our trusted medical social workers bridge the gap between you, your family and community resources to improve your recovery. Social workers work closely with patients and families to provide the social, economic and emotional solutions that will help lead to greater health and independence.

      From referrals to counseling to financial assistance, our medical social workers provide answers, including:

      • Assessments of your social, economic and emotional needs relating to your overall medical plan.
      • Education about and connection to appropriate resources and support in our community.
      • Assistance with your transfer to a more appropriate home setting if needed (for example, to an apartment, retirement community, residential care facility or nursing home).
      • Assistance with planning and decision-making assistance in various situations related to your illness, injury or overall medical plan.
      • Patient and family counseling, including adjustment and management of your condition.

      Private Duty Nursing

      If you are caring for a loved one who needs compassionate nursing care at home, you can turn to ChristianaCare HomeHealth for help. Whether your loved one needs help during the day or night, this expert care is available for medically fragile children and complex adults. Our highly skilled staff can assist with many different facets of medical care, including tracheostomy maintenance, ventilator support, gastric tube feedings, giving medications and much more. We work with you, your loved one’s doctor and other health care providers to create a care plan that will meet your needs.

      In-Home Care

      Our experienced nurses will provide expert care in your home, school or any place in the community where your loved needs it. Our goal is to help your family member live a full and meaningful life, and offer respite and peace of mind for you, the caregiver.
