Partnering with Pennsylvania
ChristianaCare has been serving neighbors in southern Chester County since 2020 with primary care in three practices located in Jennersville, West Grove and Kennett Square. Combined, these three practices have been the “medical home” for 22,000 residents in these communities.
ChristianaCare is grateful to our Pennsylvania community partners for their feedback and support as we developed our West Grove campus. This project is funded in part through generous grants from the Commonwealth of Pennsylvania and from the Chester County Board of Commissioners - Josh Maxwell, Marian Moskowitz and Eric Roe.
Community Health Needs Assessment
ChristianaCare must conduct a Community Health Needs Assessment (CHNA) every 3 years. The CHNA takes into account expertise from community leaders, public health professionals, and community members. This assessment also includes an analysis of primary and secondary data collected from local, state and national health resources.
ChristianaCare welcomes comments and questions about our Community Health Needs Assessment and/or Community Health Implementation Plan. These, and requests for paper copies of the documents, can be emailed to communitybenefit@christianacare.org.
