Managing chronic conditions through improved nutrition and food access
The Healthy Food Delivery Program is a six-month Food is Medicine initiative designed to help ChristianaCare patients better manage chronic conditions through improved nutrition and food access.
Through a partnership with Hungry Harvest, a mission-driven online grocer dedicated to reducing food waste and fighting hunger, participants receive bi-weekly home deliveries of healthy groceries, along with virtual nutrition education and customized healthy recipes to support dietary change.
Patients select their groceries through a Client Choice Virtual Marketplace, featuring fresh produce, whole grains, low-fat dairy, canned proteins, and snacks that meet specific nutritional standards.
If patients do not make selections during their ordering window, a curated box of medically supportive foods is automatically delivered, ensuring continuous access to healthy options.
Hungry Harvest sources much of the food by rescuing surplus or cosmetically imperfect items, aligning sustainability efforts with improved patient health.
The program not only addresses clinical needs but also screens for social drivers of health (SDOH), connecting patients to additional community resources to support long-term health outcomes.
