Why Choose ChristianaCare?

The ChristianaCare Anticoagulation Center cares for patients who are on an anticoagulant medication (blood thinner), such as warfarin (Coumadin) to prevent blood clots or stroke.

The Anticoagulation Center is staffed by specially trained clinical pharmacists who work with the Cardiology Consultants providers to make sure patients receive safe and optimal care. The clinical pharmacists perform international normalized ratio (INR) lab monitoring, adjust warfarin doses per protocol as necessary, conduct follow up lab work, review all your medications to prevent or monitor drug interactions, and provide education and counseling for patients taking anticoagulants.

We offer comprehensive face-to-face visits for fingerstick INR testing by appointment. We also manage patients via telephone who test at home or at a lab.

Referral

Patients must be 18 years or older and already on warfarin or will need to begin warfarin. Eligible patients are referred by a ChristianaCare Cardiology provider.
