Veloxis Patient Assistance Program

Most insurance companies do not cover the commonly prescribed post-transplant medication, Veloxis. The Veloxis Patient Assistance Program provides financial support for patients who meet certain criteria. Complete the patient enrollment form at the link above to see if you qualify.

National Living Donor Assistance Program

The National Living Donor Assistance Program assists some living donors with financial support to cover the costs of transportation and lodging for donor evaluation, surgery, and follow-up care and loss wages and childcare expenses. Information is available in English and Spanish. This application involves the donor, recipient and transplant Social Worker.

Aging with Dignity: Five Wishes

Families also use Five Wishes to help start and guide family conversations about care in times of serious illness. Five Wishes is helpful for all adults – everyone over 18 years old – and anyone can start the conversation within a family. Sometimes it begins with grandparents and other times it is the younger family members who bring up the topic. Regardless of your age, you can bring this gift to your family.

PAN Foundation

Patient Access Network is also known as the PAN Foundation, is where patients can go to apply for co-pay assistance for medications. Or the you can call for English and Spanish: 1-866-316-PANF. These applications can be initiated by the patient, but the social worker must complete a portion with medical information.

Phosphorus is a mineral found in almost all foods. It works with calcium and vitamin D to keep your bones healthy. Healthy kidneys keep the right amount of phosphorus in your body. When your kidneys are not working well, phosphorus can build up in your blood. Too much phosphorus in your blood can lead to weak bones that break easily.

My Transplant Coach is an interactive decision aid to help kidney patients learn about and understand their condition, and assist them in making healthcare decisions. Part of the family of educational programs available through Explore Transplant, My Transplant Coach is an interactive, online, video-based decision aid for patients with kidney failure. It uses animated videos and personalized graphs to help patients understand what treatment options are available, so they can make informed decisions that are right for them.

Living Well with Kidney Failure

Living Well with Kidney Failure is an educational video series on DVD with six companion booklets in English, Spanish, and close-captioned text that helps you educate patients and their families about kidney failure and its treatment.
