Nominate a Nurse-led team or group of nurses who have worked together to identify and meet patient and/or patient family needs by going above and beyond the traditional role of Nursing.

The DAISY Award - For Extraordinary Nurses, in memory of J. Patrick Barnes

The team must be led by a direct-care nurse.
The team must be comprised of two or more team members.
All team members must exemplify ChristianaCare’s values and mission.
All team members are role models of collaboration and teamwork that make a difference in the lives of patients, families, colleagues and the community.
