Remote Patient Monitoring Guide

Providing your care team with information to better manage your care.

Important Note

Welcome to this exciting program! Remote patient monitoring helps provide your care team with more information to better manage your care. You may receive automated alerts and notifications based on readings. This is NOT AN EMERGENCY SERVICE and results are not monitored in real time. If you are having any symptoms or medical problems, you should contact your physician or call 911.

Enrollment & Initial Setup

  1. Download the HealthBridge mobile application from your enrollment invitation e-mail. The email will be from <noreply@validic.com> (check your spam folder if you do not see it).
  2. Open the HealthBridge mobile application and click <Register> (bottom left corner).
  3. Enter your email address and date of birth and click <REGISTER>.
  4. You will be sent a PIN code to verify.
  5. Enter the PIN and click <VERIFY>.
  6. Create a password.
  7. Check the consent box, and click <CREATE>.
  8. Click <START YOUR PROGRAM> under To Do.
  9. Review program details and consent and click <I AGREE>.
  10. Begin connecting your devices (see next section).

Connecting the Devices

Follow the “To Do” instructions in the HealthBridge mobile application to CONNECT your device(s):

  • Make sure to choose the right device and model number.
  • Follow the on-screen instructions for your device to PAIR/CONNECT.
  • Confirm that Bluetooth is enabled and PAIR/CONNECT your device.
  • You will be asked to take a reading to complete and test your connection.

    Detailed Instructions for Connecting Devices

    Using the Devices to Take and Send Measurements

    • Please follow the recommendations of your care team or monitoring program for how often and when to check your vitals.
    • Keep the HealthBridge mobile application OPEN and running in the background – DO NOT fully close/exit.
    • Ensure that your smartphone has a strong Wi-fi or Cellular connection and that Bluetooth is enabled.
    • Ensure that your smartphone is in close proximity to your devices.
    • The HealthBridge mobile application will show all synched readings. SWIPE DOWN on the screen to refresh.
    • Take readings with your devices and track your progress over time.
    • Readings automatically and securely sync to your care team.
    • You may receive text messages after submitting results and/or reminders if you forget to submit.
    • Preparing to take your blood pressure:
      • Don’t drink caffeine, smoke, or exercise for at least 30 minutes before you take your blood pressure.
      • Empty your bladder before the test.
      • Sit with your back straight, such as on a dining chair, with both feet on the floor.
      • Rest quietly for at least 5 minutes before you take a reading.
      • Do not move, talk, or text while you take your blood pressure.
    • Placing the cuff:
      • Sit with your arm slightly bent and resting comfortably on a table so that your upper arm is on the same level as your heart. Use the same arm each time you take your blood pressure.
      • Wrap the blood pressure cuff around your bare upper arm. The lower edge of the cuff should be about 1 inch (2.5 cm) above the bend of your elbow.
    • We currently support the following blood pressure cuffs in our program:
      • A&D UA-651BLE Blood Pressure Monitor.
      • Omron 5 Series (BP7250) Upper Arm Blood Pressure Monitor.
      • Omron 7 Series (BP7350) Wireless Upper Arm Blood Pressure Monitor.
      • Welch Allyn RPM-BP100 (Series 1500) Blood Pressure Monitor.
    • To start measuring your blood pressure, enroll and pair your device to the HealthBridge mobile application.
    • For detailed instructions, click here.
    • In general, we recommend that you check your blood sugar at least 2 times per day and at different times throughout the day. Alternate when you check and make sure to record on your device.
      • Fasting (in the morning before eating).
      • Before meals (breakfast, lunch, dinner).
      • After meals (check 1-2 hours after starting your meal).
    • We currently support the following blood glucose monitoring devices in our program:
      • Accu-Chek Guide.
      • Ascencia Contour Next One.
      • Verio One Touch.
    • To start measuring your blood glucose levels, enroll and pair your device to the HealthBridge mobile application.

    If You're Having Problems with Device Connectivity or Data Uploading

    General Information
    • The HealthBridge mobile application must be open.
    • Ensure that you are connected to WiFi or Cellular Data (e.g. 4G LTE or 5G) and that you have a strong connection.
    • The remote monitoring device should be in close proximity of your phone or tablet.
    • Vital readings may take time to upload; Swipe down on the screen to update.
    Additional Troubleshooting Tips
    • Smartphone/device shows that my monitoring device(s) are not connected:
    • DO NOT MODIFY Bluetooth settings from the device or try to force the device to pair. The device may show it is not connected, but that is expected when not actively sending data
    • Are you using an Android device? Please refer to these additional tips:
      • Keep the HealthBridge mobile app open and active while taking measurements.
      • Disable Android battery optimization for HealthBridge app.

    • If you are still having problems, remove and re-connect your medical device(s).
      • Remove the device(s) from the HealthBridge mobile application connection profile
      • Remove the device(s) from the Bluetooth settings on your cell phone.
      • Re-connect your device(s).
