Welcome to this exciting program! Remote patient monitoring helps provide your care team with more information to better manage your care. You may receive automated alerts and notifications based on readings. This is NOT AN EMERGENCY SERVICE and results are not monitored in real time. If you are having any symptoms or medical problems, you should contact your physician or call 911.
Enrollment & Initial Setup
- Download the HealthBridge mobile application from your enrollment invitation e-mail. The email will be from <noreply@validic.com> (check your spam folder if you do not see it).
- Open the HealthBridge mobile application and click <Register> (bottom left corner).
- Enter your email address and date of birth and click <REGISTER>.
- You will be sent a PIN code to verify.
- Enter the PIN and click <VERIFY>.
- Create a password.
- Check the consent box, and click <CREATE>.
- Click <START YOUR PROGRAM> under To Do.
- Review program details and consent and click <I AGREE>.
- Begin connecting your devices (see next section).
