Your Upcoming Office Visit

We look forward to seeing you at your upcoming appointment.  To keep you and your care team as safe as possible, review the information below.  Please call your provider’s office if you have any questions or concerns.  Learn more about how we will keep you safe during your visit.

Screening – we will reach out before your visit to ask a few simple screening questions about current respiratory symptoms.

Masks – masking is optional.  However, anyone with respiratory symptoms (coughing, sneezing, etc) should wear a mask at all times. We will provide a mask to you or your caregiver if you request one.

Caregivers – Only patients needing help can have a caregiver with them for their visit.

Medications – Bring a list of any medicines you are currently taking to your visit, including:

• Dose information (example: 50mg)
• Frequency (example: twice a day)
