GUIDE Memory Program

Additional dementia care services at no additional cost.

The ChristianaCare GUIDE Memory Program may offer you more dementia care services at no additional cost.

Starting July 2025, the Centers for Medicare & Medicaid Services (CMS) will start to enroll patients in GUIDE programs to provide ongoing dementia care services along with caregiver support.

CMS selected ChristianaCare to offer a GUIDE program as part of a new nationwide initiative to:

  • Enhance the quality of life for people living with dementia.
  • Help people with dementia remain at home longer.
  • Reduce the burden on unpaid caregivers who are often family members.
The ChristianaCare GUIDE Memory Program offers:
Comprehensive Care

Expert diagnosis and a personalized treatment plan along with ongoing care coordination and support to meet the needs of each patient and their caregiver.

Why choose the GUIDE Program?

The ChristianaCare GUIDE program is part of our Swank Center for Memory Care. As a program participant, you receive all the services our Swank Center can provide for the treatment of dementia along with support for your caregiver. Our GUIDE participants have access to expert clinicians and the latest Alzheimer’s treatment that distinguish the Swank Center as a leader in memory care.  

Our multidisciplinary team of clinicians includes board-certified physicians in geriatrics, geriatric psychiatry, and neurology as well as specialized social workers, nurse practitioners and nurse educators. ChristianaCare clinicians participating in the Swank Center are here for all your health needs.

  • Comprehensive assessments
  • Personalized care plans
  • Medication management
  • Monitoring and support
  • Care coordination across health care services and doctors
  • Caregiver education, training and support
  • Connection to community and peer support group

Additional benefits for GUIDE participants will include after-hours support and respite care. These services will be available when GUIDE goes live in July 2025. 

GUIDE Beneficiary Eligibility Criteria

Dementia Diagnosis

Beneficiary has dementia confirmed by attestation from clinician practicing within a participating GUIDE dementia care program

Enrolled in Medicare Parts A & B

Beneficiary must have Medicare as their primary payer and not enrolled in Medicare Advantage, including Special Needs Plans (SNPs)

Please call or complete the form below if you have questions about eligibility. Individuals with Medicare Advantage can use the services of the Swank Center for Memory Care even though they cannot take part in a GUIDE program.
