Nearly one-third of low-income households are food insecure and struggle to put healthy food on the table.

This food insecurity drives the consumption of accessible, cheaper, less nutritious food, increasing the risk of developing nutrition-related health conditions such as:

Many low-income, food-insecure individuals who have been diagnosed with a chronic disease experience worse health outcomes because they often need to make trade-offs between important basic needs, such as housing or medical bills, and purchasing nutritionally adequate foods.  Produce programs that provide free or low-cost fruits and vegetables offer an effective solution to these social barriers.

The Produce Delivery Program, in partnership with Urban Acres, was implemented to increase access to healthy foods for our patients, especially during the COVID-19 pandemic when we’ve been seeing a rise in food insecurity. In addition to being connected with a community health worker, our eligible patients get produce delivered directly to their homes once a week. Ultimately, it is our goal to improve our patients’ health outcomes by increasing access to fresh food and empowering them to make healthier food choices.
