Survivorship Care Is a Priority in Cancer Care

Whether it was 10 months ago or 10 years ago, you still remember when the doctor said you were done with your cancer treatments. Now, you just want things to stay that way. The childhood cancer specialists at the Helen F. Graham Cancer Center & Research Institute want nothing more than to make that happen.

As a survivor of childhood cancer, our team of specialists will work with your primary care physician to support your overall wellness, including emotional support, disease prevention and rehabilitation. Don’t have a primary care physician? No problem. We can help with that, too.

Depending on your current health, we may also refer you to additional medical specialists. Then your entire multidisciplinary team will work together with your primary care doctor to create the best plan to care for your unique needs and navigate life after childhood cancer.

Easing the Transition From Active Cancer Care to Long-Term Health

The Adolescent/Young Adult Cancer Survivor Program will review your medical history with you and your doctors and assess long-term health risks that include heart, lung, and kidney problems as well as any hearing, vision, or cognitive issues. They can also discuss any implications about fertility or secondary cancers.

Your Visit With Your Care Team

Your care team appointment may include the following:

  • A physical exam and review of recent medical history
  • Assessment to detect cancer recurrence or secondary cancers
  • Assessment of the physical and emotional effect of cancer treatment
  • Review of your survivorship treatment summary and care plan
  • Referral for specialty medical disciplines as needed
  • Education on nutrition, exercise, smoking cessation, and emotional well-being for a mind, body, spirit approach to health care.

The program has appointments available at both Wilmington Hospital and the multidisciplinary clinic at the Graham Cancer Center. At the Graham Cancer Center, we offer an approach to care with all of your providers and services offered in one place.
